Audrey was married twice in her life. Her first marriage was with American actor Mel Ferrer, whom she met at a party hosted by her friend Gregory Peck. Mel later sent her a script for the play Ondine, and Audrey agreed to play a role in it. They started to rehearse the play in January 1954 and Audrey and Mel married in September that year. Despite the gossip columns saying that the marriage wouldn't last, Audrey said they were very happy together, but she did admit that he had a bad temper. Their only child, Sean, was born in 1960, but Audrey had two miscarriages before that, one in March 1955 abnd one in 1959 when she fell of a horse during the filming of The Unforgiven, where she also broke her back. After spending weeks in hospital, she lost her baby because of physical and mental stress. She had another miscarriage in 1965 and was very careful when she was pregnant with Luca in 1969-1970. Her fourth and final miscarriage was in 1974.
She and Mel divorced in 1968 and Audrey said that she had stayed in the marriage for too long. Both Mel and Audrey was rumored to have had affairs on the side for the final years of their marriage, Audrey with her Two For The Road co-star Albert Finney.  Mel and Audrey separated before their divorce and Audrey lost weight during the separation.

She met Italian psychiatrist Andrea Dotti 1968 on a cruise.  They married in January 1969 and had a son together in February 1970. Andrea loved Audrey and was also liked by her son Sean, but began having affairs with other women. Their marriage ended in 1982, when Audrey's sons were old enough to handle living with a single mother.
She only spoke twice to Mel in her life, at Sean's graduation and first wedding, but she stayed in contact with Andrea, for Luca's sake.

She spent the final years of her life with Dutch actor Robert Wolders, who she met through a friend in the last years of her marriage to Andrea. She called her years with him the happiest of her life, though they never married.
